We help our clients determine the level of care that is required and which nursing professional will best meet your needs and those of your family or loved one. To us, nothing is more important than helping  individuals live full and dignified lives while ensuring they maintain the highest possible level of independent living.

Our Registered Nurse Case Manager provides a free in-home assessment and consultation. We assess risk and manage the safety and security of each client in the home. When we place our staff we observe strict screening protocols to ensure an optimal match for each client. We also make regular review visits and are always on call with 24-hour telephone support.

We conduct regular on-site visits to ensure quality and consistency, and offer 24-hour 'on-call' support for both clients and staff.

We provide a care environment that respects the dignity, spiritual needs and cultural practices of each individual, and promotes independence and choice. Whether for long or short term care, post hospitalization follow-up, assistance with daily living - or companionship, our team is always at your disposal.

Specialized Nursing

Palliative care:

Our staff are specialized in palliative care, which strives to meet the expectations and needs of the person whose illness no longer responds to treatment aimed at a cure. Palliative care does not hasten or delay death. The mission of our palliative care team is to provide as much freedom as possible from suffering by giving physical, emotional, mental and spiritual care as well as comfort and compassion to the patient, and the patient’s loved ones.

  • Alzheimers and other dementias
  • Parkinsons, stroke, ALS, multiple sclerosis and other disorders
  • Chronic diseases: cancer, chronic obstructive pulmonary disease (COPD), diabetes, high blood pressure, congestive heart failure

Caregiver Relief, Companionship and Live-In

We provide as much or as little care that is needed to remain safe and independent at home. Our fully qualified service providers help with personal care and provide companionship, activities, light housekeeping and transportation. We also provide caregiver relief services to those who are caring for a loved one. Your needs, whether you are a client or a family caregiver, determine the hours and type of care we provide, whether you need an hour a week or 24/7 round-the-clock support.

Case Management

Case management involves the coordination of health-related services for patients by allocating a professional to be responsible for assessing needs and implementing care plans. Excel Santé | Health case managers develop individualized plans and coordinate services to help meet each patient’s healthcare needs. We focus on situations when the patient has a condition requiring multiple services from multiple providers, and also in situations when the patient’s family is not available and close by.

Blood Test Services

Excel Santé | Health provides a mobile nurse service to administer your blood tests. The results are forwarded directly to your doctor.

We give training in subjects as palliative care, Principles for Moving Safely (PDSB), cardio-pulmonary resuscitation (CPR), automated external defribillation (AED), administering diabetes injections safely, wound care, foot care, nutrition, and others.
